Working with the resin of Indian hemp in a laboratory in\nBoston, Massachusetts, they isolated and named the first cannabinoid\ncompound, known as cannabinol (CBN). Their discovery was one of many\nthat led to a paradigm shift in research into human health and disease.</p>\n<h2>One Discovery Leads to Another</h2>\n<p>In 1940, more than four decades after the discovery of CBN, Roger Adams\nisolated and identified another cannabinoid, cannabidiol (CBD), while\nworking with red oil extract made from Minnesota wild hemp at the\nUniversity of Illinois. After Adams's discovery, it was 24 years until\nthe next breakthrough in cannabinoid science.</p>\n<p>Working at Hebrew University in Jerusalem, Raphael Mechoulam identified\ndelta-9-tetrahydrocannabinol (THC), the substance responsible for much\nof the psychoactive, or mood-altering, effects of cannabis. Since the\ndiscovery of THC, there have been rapid advances in the isolation and\nidentification of cannabinoids. So far, <a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2866040/\">over\n85</a> unique\ncannabinoids have been identified.</p>\n<h2>Further Breakthroughs in Cannabinoid Science</h2>\n<p>As researchers continued to learn more about cannabinoids, it appeared\nincreasingly likely that the effects of cannabinoids must be mediated by\na family of receptors. It was not until 1988 that the first of these\nreceptors, <a href=\"http://www.drugscience.org/Petition/C3D.html\">cannabinoid receptor type\n1</a> (CB1) was revealed by\n<a href=\"http://www.ncbi.nlm.nih.gov/pubmed/2848184\">Allyn Howlett</a> and William\nDevane of St. Louis University Medical School, Missouri.</p>\n<p>Since then, CB1 receptors have been found in large numbers in several\nareas of the human brain, including the basal ganglia (affecting motor\nactivity), neocortex (thinking), hippocampus (short-term memory),\ncerebellum (motor coordination), periaqueductal gray (pain perception),\nand hypothalamus and limbic cortex (appetite and sedation).</p>\n<p>These receptors are also found to a lesser extent in the dorsal horn of\nthe spinal cord (affecting pain perception) and on immune cells. A few\nyears after the discovery of CB1, a <a href=\"http://www.gwpharm.com/Research%20History.aspx\">second\nreceptor</a>, cannabinoid\nreceptor type 2 (CB2), was identified by researchers <a href=\"https://www.ncbi.nlm.nih.gov/pubmed/7689702\">Munro, Thomas and\nAbu-Shaar</a> from the MRC\nLaboratory of Molecular Biology in Cambridge, United Kingdom. CB2\nreceptors are predominantly found in immune cells and tissues, with the\ngreatest density in the spleen. Activation of these receptors reduces\ninflammation, a precursor to many diseases.</p>\n<h2>Discovery of a New Molecular Signalling System</h2>\n<p>The discovery of cannabinoid receptors fueled speculation that the body\nmust produce compounds that bind to the receptors. The first such\ncompound, anandamide (AEA), was isolated by <a href=\"http://www.sciencemag.org/content/258/5090/1946\">Raphael\nMechoulam</a> and his team\nat Hebrew University in Jerusalem in 1992.</p>\n<p>A couple of years later, a second compound, 2-arachidonoylglycerol\n(2-AG), was discovered and in the following decade several other\ncompounds capable of activating the cannabinoid receptors were\nidentified. These compounds, together with the receptors, make up the\nendocannabinoid system.</p>\n<h2>Manipulation of the Endocannabinoid System</h2>\n<p><a href=\"http://emedicine.medscape.com/article/1361971-overview#%20a4\">Research</a>\nindicated that the <a href=\"http://norml.org/library/item/introduction-to-the-endocannabinoid-system\">endocannabinoid\nsystem</a>\nmediates a number of physiological functions vital for survival. More\nimportantly, it suggested that modulating the activity of the\nendocannabinoid system using cannabinoids may be the key to treating a\nbroad range of diseases and symptoms.</p>\n<p>Currently, THC and CBD are the two main cannabinoids of medical\ninterest.\n<a href=\"http://www.drugabuse.gov/publications/drugfacts/marijuana-medicine\">THC</a>\nhas been found to increase appetite and reduce nausea, and may help\ndecrease pain, inflammation, and muscle control problems. CBD may be\nuseful in controlling epileptic seizures, reducing pain and\ninflammation, and quelling mental illnesses and addictions.</p>\n<h2>Researchers Hone Synthetic Cannabinoids</h2>\n<p>Continued research into the therapeutic effects of cannabinoids led to\nthe creation of synthetic cannabinoids -- chemicals created to mimic the\neffects of natural cannabinoids. Two THC-based medications,\n<a href=\"http://www.nlm.nih.gov/medlineplus/druginfo/meds/a607054.html\">dronabinol</a>\n(Marinol) and\n<a href=\"http://www.nlm.nih.gov/medlineplus/druginfo/meds/a607048.html\">nabilone</a>\n(Cesamet), showed clinical benefit and were subsequently approved by the\nU.S. Food and Drug Administration (FDA) for the prevention or treatment\nof chemotherapy-induced nausea and vomiting. Other cannabis-based\nmedications are currently in development.</p>\n<h2>Combined Approach Offers Greater Hope</h2>\n<p>Research has yet to reveal the exact mechanisms by which all of the\ncannabinoids in the cannabis family of plants exert their therapeutic\neffects, but evidence suggests that these compounds work better together\nthan alone: a discovery termed the entourage effect. Support for this\neffect derives from comparisons of the effect of the whole cannabis\nplant with that of the drug Marinol. When Marinol became available as a\nmedicine in the mid-1980s, researchers believed that it would have the\nsame effect as the entire cannabis plant. However,\n<a href=\"http://www.coloradonorml.org/information/marijuana/medical/marinol-vs-natural-plant\">patients</a>\nprescribed Marinol frequently reported greater psychoactive effects --\ncaused, in part, by the lack of CBD, which is believed to modulate the\npsychoactivity of the THC.</p>\n<h2>Advocates Call For Overhaul of Marijuana Laws</h2>\n<p>The ability of cannabinoids to work together to treat a variety of\nailments while causing minimal adverse side effects has led to calls for\nmedical marijuana --the use of the cannabis plant or its basic extracts\nfor medical purposes -- to be legalized. <a href=\"https://www.whitehouse.gov/ondcp/state-laws-related-to-marijuana\">Twenty-three\nstates</a>\nand Washington, DC, have already passed laws allowing marijuana to be\nused for a variety of medical conditions.
