I have some health issues including essential hyperteinsion, depression, insomnia. I was diagnosed with MONO in 2016.\n\nFor the past 2 years I have tried medications that were either sleep meds or psychiatric meds to help with sedation.  Some helped for awhile, but my body eventually developed a tolerance.  In general, my tolerance for meds is LOW.\n\nCurrenly I am using a Dosist Sleep Pen at 9 PM and I do get high from 1 dose.  However, by 11 PM, I am still not sleepy at all.\n\nIn the past, I have tried 8:1 and 1:1 CBD Subliminal Tinctures, 18:1 CBD Gel Caps, PLUS Restore Gummies.  None have helped significantly with sedation.\n\nI do my best to be active and work my body.  I do not take any recreational drugs or use any alcohol.  I try to observe sleep hygiene and practice mindfulness.  \n\nMy general health is good.  My PCP examined me last fall.  My current meds include a statin, blood pressure, vitamins, and baby aspirin.  \n\nI am financially stable, have rich history of family and friends, am divorced since 2013.  I am working on lifestyle changes, but it is slow going.  Since 2005 I worked with seniors  in Senior Communities. I have been working on a spiritual practice, recently learning what Ananda has to offer.\n\nAny comments or suggestions are much appreciated.\"\n_____________________\nLook at the strain of cannabis you are using as well.  You should try to aim for an indica strain which has a more of a sedating and relaxing effect.  Sativa is more energizing and better used in the day time.  You should look for a product that has a mid to high range THC (if no contraindications) and I would advise you dose in an oil form as the effect will last longer but of course the onset is much slower than inhalation.  Follow sleep hygiene rules and you don't want to develop a sleep dependency issue with any sleep agents, and lastlly I would advise that you consider a referral for a sleep study from your primary care provider.  \n\nAmyT","image":null}},{"node":{"id":"Question_12320-markdown","title":"Struggling with side effects","slug":"struggling-with-side-effects","content":"\"I was instructed by my psychiatrist to start CBD oil on top of my Cymbalta because I can’t tolerate going up anymore on the dosage. First weeks of using CBD from reparable brands I felt great. Lifted my mood, gave me energy, could take multiple dosages of it a day and felt fine. But now it’s making me really sleepy, even at low dosages, I’m talking about less than 5 mg here. I take it sublingually. It’s full spectrum CBD too. If I stop taking it, the low mood will start coming back, so it is working for that but the triedness is becoming a problem. It was working so good that I felt like I’d be able to reduce my antidepressant one day. Now I feel I can’t tolerate the CBD oil. Btw, if I take it at night, I either have where I’m tired the next day or sometimes (and this is the weirdest thing) it’ll wake me up. Still is all very frustrating. \"\n_____________________\nHi there! I think it would be a great idea to discuss your experience, and tapering off of the Cymbalta, with your psychiatrist. However, that 8:1 product would be an  excellent choice to start off with. Each dropper-full should contain  approximately 16mg of CBD and 2mg of THC. To avoid getting high and  forgetful, I would likely recommend that you start with 1/2 of a dropper  full (about 10 drops) which would provide 1mg of THC and 8mg of CBD. Any information would be greatly appreciated, my thanks in advance.\"\n_____________________\nWhat you describe sounds like the beginning of a \"fainting spell\" from which you recovered before the actual faint.  In otherwise healthy individuals this is usually a transient episode of low blood pressure which resolves on its own when the symptoms force the patient to sit or lie down.  Dehydration is often a contributing factor, and drinking water was probably an excellent idea.  Cannabis itself has no consistent effect on blood pressure, but once the mind and emotions enter the picture changes in heart rate and other significant cardiovascular responses can happen secondarily.  If you do not have any current health problems and are not taking any medications I would not expect this to happen again. These benefits include reduction of spasticity, improvement of urinary symptoms, reduction of pain, improvement of mood.  The medical literature is full of small studies that demonstrate these effects (https://scholar.google.com/scholar?q=cannabis+multiple+sclerosis+study&hl=en&as_sdt=0&as_vis=1&oi=scholart&sa=X&ved=0ahUKEwirhP_V6PPYAhVL2mMKHaIrBkgQgQMIJzAA), although the effects are not always profound.  I believe THC is extremely important in producing these effects, and the presence of sufficient CBD is also important.  I do not think there is yet clinical science supporting a particular ratio of CBD:THC, but 1:1 is probably a good place to start, varying the ratio and amount taken to discover what is most helpful. Midazolam is a benzodiazepine (like Xanax) used to help patients with anxiety, sleep, and it's also used in many operating rooms as it also allows for some anterograde amnesia.  Benzodiazepines and cannabis, in my observation over the years, seem to have a bi-phasic response.  In low doses, both can be anxiolytic (decrease anxiety) but it high doses, in some patients there is a paradoxical increase in fear response;  to me, this is what sounds like is happening.  For safety's sake, I never recommend my patients use medical cannabis products and anxiolytics like Dormicum/ Midazolam at the same time.  I also recommend other modalities to help with anxiety reduction that are not drug/ cannabis based (i.e. vigorous exercise daily,  meditation/ mindfulness practice,  CBT/ cognitive behavioral therapy) through an MD referral, etc). Both are sativa-dominant hybrid strains that are great for lifting depression and enhancing feelings of well-being. I use Relax at night to help me sleep when I'm in pain, or just stressed out and I'm able to get up in the morning no problem. I also have Inspire but I find that I use it less than the others, it's just not my jam. It's a great way to dip your toe into cannabis treatment for mental health I've found because the low doses are easier to track and control. You're not all-in all of a sudden. It's very gentle. \r\n\r\nAs for pain, I've recently discovered these transdermal patches by Mary's Medicinals. You put them on the inside of your wrist during flare ups and it really helps with spasms and muscle pain. They last for 12 hours and are pretty amazing. You don't feel high, it just goes to work on your pain, much the way anti-inflammatories might. The patch is skin-colored (if you're white) and can also be placed on the ankle. The combination of the Alchemy low-dose pens and the transdermal patches has been such a winner for me when it comes to depression and pain. \r\n\r\nI hope you get to feeling better really soon, depression is the worst, and you should pat yourself on the back for reaching out and doing some good self-care.
