Cytochrome P450 help metabolize most drugs, including Lorzapem. All cannabinoids affect the Cytochrome P450 enzymes, as well as other enzymes in the body, but CBD affects it the most. \n\nResearch in this area is still pretty scant, so the level that CBD inhibits these enzymes is not totally understood. It is clear though that higher doses of CBD may affect this cluster of enzymes more. \nCBD will probably affect how your body metabolizes Lorezapem, but it’s not clear exactly how. \n\nIt’s a good idea to talk to your doctor about using CBD in conjunction with other drugs. Drug interactions vary wildly from person to person, so the best way to figure out which combination of drugs work best for you is work closely with your health care professional. \n\nErik McLaren\n_____________________\nI find this a helpful bacground/primer regarding the use of cannabis and benzodiazepines. THC will be helpful but CBD is what will help to put autoimmune disease in remission and the THC may make your friend happy and functional. I would start your friend on a high dose CBD tincture to start building up the CBD levels. The THC may be done during the day (Please, see the article on microdosing THC) with CBD at night for deep restorative sleep and long term improved health but it will take about 1 month before there is a noticable difference and two months before you can really tell a difference so please, trust it will work. \nDo consider going on a Paleo diet to eliminate all free glutamates from the diet since these are highly addictive and toxic especially to the neurological system. Have you tried more than 10mg of the Indica edible? I would suggest trying a High THC indica tincture, you can start with a low dose and increase until you get the desired result, you might need more than 10 mg. Some people are very sensitive to THC and only need 2-3mg while others might need 15mg...everyone is different so there is no one size fits all. This is rarely clinically significant, but occasionally it can be important, especially if other prescription medications are being taken that share the same metabolic route. I don't believe being on Coumadin is a contraindication to medical cannabis use, but patients who are using cannabis regularly should have their INR checked regularly as well to make sure their level of anticoagulation is appropriate.\n\ndrelkind\n_____________________\nThe cytochrome p450 system accounts for 70 to 80 percent of the enzymes involved in drug clearance, making it the most important system for drug metabolism. Both THC and CBD are competitive inhibitors of three different enzymes in the cytochrome p450 system, two of which are involved in the metabolism of coumadin. This means that CBD and THC are competing for the same receptors as is coumadin. So when coumadin is taken concurrently with CBD and/or THC, coumadin levels could possibly rise. When coumadin levels rise excessively, blood clotting may be compromised leading to signs ranging from frequent bloody noses all the way on up to a potentially fatal event, such as a hemorrhagic stroke or heart attack.\nhttps://ghr.nlm.nih.gov/primer/genefamily/cytochromep450\n\nA case report published in 2009 described a patient who was on coumadin therapy for eleven years. He was admitted to the hospital for a severe gastrointestinal bleed when he confessed to consuming much more cannabis than was typical for him; his INR was 10.41 (therapeutic range is usually between 2 and 3); after inpatient treatment, his INR normalized; then, fifteen days after being discharged, he was readmitted with an INR of 11.55; after he quit using cannabis, his INR remained between 1.08 and 4.40 and he suffered no further complications.  \nhttps://www.ncbi.nlm.nih.gov/pubmed/19531696\n\ndrkim\n_____________________\nAnother case report suggested that extremely high doses of CBD can lead to markedly elevated coumadin levels. Enrolled in a clinical trial evaluating CBD as a potential treatment for epilepsy, the test subject was given extremely high doses of CBD. He was first given doses averaging 265mg, which resulted in a slightly elevated coumadin levels; at doses averaging 528mg, the coumadin levels were markedly elevated; after reducing the coumadin dose, INR results normalized. Relevantly, very few patients, if any, would ever take as much CBD.\nhttps://www.ncbi.nlm.nih.gov/pmc/articles/PMC5789126/\n\nHowever,as previously mentioned, a highly elevated coumadin level can lead to serious consequences. And because the effects of cannabis on coumadin levels have yet to be sufficiently studied, as Dr. Elkind advised, you should discuss the matter with your prescribing physician and monitor your INR results with additional vigilance.\n\ndrkim","image":null}},{"node":{"id":"Question_8981-markdown","title":"What is the best form of CBD oil to use in chemo for Stage 2 breast cancer?","slug":"what-is-the-best-form-of-cbd-oil-to-use-in-chemo-for-stage-2-breast-cancer","content":"\"I tried some in dropper form but it caused a lot of abdominal cramping and gas.\"\n_____________________\nHi there!
