We spoke to HelloMD’s Dr. Richard Kim to discuss SAD and how to use cannabidiol (CBD) to help modulate its symptoms.\n\n### Seasonal Affective Disorder: A Serotonin Issue\n\n“The causes of SAD aren’t fully understood. However, recent research suggests that people with SAD regulate serotonin differently in comparison to others,” Dr. Kim notes. \n\nFolks with depression usually understand the important role of the serotonin reuptake system in the brain. Those with an improperly functioning serotonin reuptake mechanism may be more vulnerable to seasonal depressive symptoms. \n\n“In a 2016 study, researchers found that those with SAD had increased serotonin transporter (SERT) activity during the autumn and winter months, in comparison to those without SAD. Higher SERT activity results in lower amounts of serotonin available to bind the subtype of serotonin receptor (5-HT1A) responsible for serotonin's antidepressant effects,” Dr. Kim explains. \n\nDoctors and researchers have prescribed antidepressants for decades to help with the regulation of the serotonin system. \n\n“Restoring activity in serotonin pathways by increasing the serotonin available to bind the 5-HT1A receptor in the brain is how antidepressant medications like SSRIs (selective serotonin reuptake inhibitors) are thought to alleviate anxiety and depression. In fact, these medicines do so by inhibiting the aforementioned SERT,” says Dr. Kim.\n\n### How CBD Affects Serotonin Levels in the Brain\n\nCountless cannabis consumers have been forced to rely on anecdotal, word-of-mouth evidence to help treat a variety of conditions. For many, taking cannabis containing tetrahydrocannabinol (THC) doesn’t provide relief for depressive or anxiety-related conditions. In fact, for some, psychoactive cannabis has been found to increase levels of psychological discomfort. \n\nHowever, with the explosion in the use of hemp-derived CBD, which doesn’t make consumers experience a high like THC does, studies are confirming that [CBD can be a powerful tool for coping with both anxiety and depression](http://baltimorepostexaminer.com/how-does-cbd-oil-work-for-depression/2018/10/12).\n\n“Several recently published studies suggest that CBD reduces depression and anxiety symptoms similarly to how SSRIs do. \n\n“However, CBD does so not by increasing the serotonin available to bind 5-HT1A receptors as SSRIs do, but by mimicking serotonin's action at the receptor. In scientific terms, CBD is what's called a 5-HT1A receptor agonist,” Dr. Kim explains. \n\nA 2016 report published in _Neuropharmacology_ stated that [CBD use induced “rapid-acting antidepressant-like effects” in people suffering with depression](http://www.seattleweekly.com/food/how-weed-can-help-fight-seasonal-effective-disorder/). \n\n### Taking CBD to Ease SAD\n\nBefore beginning any cannabis-related treatments, it’s crucial to receive a correct diagnosis from your doctor. Many people don’t derive the benefits of cannabis therapy simply because they aren’t taking enough CBD to affect their condition. This is why it’s important to secure a medical cannabis recommendation from a trusted, cannabis-knowledgeable doctor like Dr. Kim.\n\n“For most patients, I recommend taking 10–20 milligrams of CBD, two to three times per day, in addition to light therapy,” says Dr. Kim. \n\n“People with SAD may also produce less vitamin D, which is also believed to play a role in serotonin activity. A 1999 study concluded that improvement in vitamin D levels was significantly associated with improvement in a patient’s depression scale. I recommend taking 5,000 international units of vitamin D3—not D2—each day,” Dr. Kim advises.\n\nIt’s also essential to use CBD products that are produced by trusted sources. You should also do your own research to determine the type of CBD product that fits your lifestyle and personal preference. \n\nFor those on the go, a single-serve candy may be the easiest way to incorporate CBD into your wellness regime. For others, a sublingual spray provides the fastest delivery system for rapid symptom reduction. The best part of experimenting with CBD therapy is that there are virtually no side effects. Depression has a number of health-based triggers such as: \n\n* Hormone fluctuations related to menstruation, pregnancy and menopause\n* Nutritional deficiencies like low levels of B vitamins\n* Certain medications\n* Neurological conditions such as Parkinson’s disease and multiple sclerosis \n* Other severe health problems including cancer\n* Genetics, since depression can run in families\n\nSocial factors can also lead to the development of depression. These include:\n\n* Traumatic and stressful life events such as the death of a spouse or family member\n* A troubled relationship \n* The loss of a job \n\nAnd people who experience traumatic events in childhood may have a greater risk of depression as adults. Depression can also appear alongside other kinds of mood disorders such as anxiety and PTSD. \n\nIn all of these situations though, the symptoms of depression generally arise from an imbalance in the brain’s systems for regulating mood. The levels of neurotransmitters responsible for positive feelings and calm such as serotonin and dopamine, or excitement and arousal like norepinephrine, may be too low or inhibited by the expression of other kinds of chemicals. \n\nBut recent studies have also shown that the endocannabinoid system (ECS), the body’s network of natural cannabinoid receptors, can play a powerful role in regulating mood and managing stress—and [this is where CBD comes in](https://www.ncbi.nlm.nih.gov/pubmed/24923339).\n\n### The Problems With Depression Medications\n\nBecause depression takes so many different forms and affects so many people, the pharmaceutical industry has developed several types of drugs to target the production of mood- regulating neurotransmitters.\n\nFor example, drugs like Paxil and Prozac are selective serotonin reuptake inhibitors, or SSRIs. They inhibit the reuptake—or reabsorption—of serotonin, so that more of it’s available to the brain. \n\nAnother class of drugs called monoamine oxidase inhibitors, or MAOIs, which includes drugs like Nardil and Marplan, block an enzyme called monoamine oxidase. This enzyme affects the levels of norepinephrine, serotonin and dopamine. \n\nBut a first round of antidepressant medication is effective for just 30% of patients. And many patients with depression have to try several different medications before finding one, or a combination of drugs, that can manage their symptoms. \n\nAnd even those antidepressants that are considered safest can have a long list of undesirable side effects such as:\n\n* Drowsiness\n* Dizziness\n* Stomach upsets \n* Weight gain \n\nSome also cause undesirable mood changes such as increased anxiety and suicidal ideation.\n\nFor those reasons, a growing number of people struggling with depression have found relief with cannabis, thanks to the potent mood-regulating properties of CBD, a cannabis compound that doesn’t make consumers feel high like tetrahydrocannabinol (THC) does, and which occurs abundantly in all strains of _Cannabis sativa_. \n\n### The Science Behind How CBD Can Help With Depression\n\nAlong with the psychoactive compound THC, and numerous other active compounds and terpenes, [CBD affects the body’s own endocannabinoid receptor system](https://www.ncbi.nlm.nih.gov/pubmed/21354188) as well as many other processes. It’s named after ananda, the Sanskrit word for bliss, and it produces feelings of well-being and happiness. \n\nBut anandamide’s effects are usually short-lived, thanks to the effects of another chemical called FAAH, which breaks it down. CBD has been shown to inhibit the effects of FAAH, so that it breaks down anandamide less quickly and allows this mood-boosting chemical to remain available longer.\n\nCBD has a long list of other healing benefits, too. CTE is characterized by neurological and physiological changes in the brain such as the buildup of tau, an abnormal protein. In CTE, tau builds up and clumps in areas of the brain where it shouldn’t be, disrupting brain functions.\n\nUnfortunately, there’s no way of diagnosing CTE while someone is alive. It can only be confirmed after death by examining the brain for tau deposits.\n\nStill, unlike Alzheimer’s disease, CTE has a clear cause—trauma from repeated concussions. First noted among boxers, and now well known as a condition that affects football players, this incurable disease can affect anyone with a history of concussions. But most of the conversation about CTE is in relation to contact sports athletes. The first line therapy is typically pharmacologic or laser therapy (not surgery). If pharmacologic therapy is chosen, one current recommendation is topical prostaglandin. If mono therapy is not sufficient, another topical (a beta blocker) may be added.\n\nFrom what we know, I would consider marijuana to be a complementary or supplemental therapy to your medication; and this decision should be arrived upon with the guidance of your ophthalmologist.
