Despite dabbing’s bad reputation, it has strong medicinal benefits. Dabs allow patients to quickly receive a strong dose of cannabinoids that is not possible with almost any other administration method. \n\n### High Strength Concentrates\n\nThe marijuana extracts, also known as concentrates, that are used in dabbing are known for their high levels of THC. However, they are also high in other beneficial components, such as other cannabinoids and terpenes. \n\nAll of the components of the plant are amplified in cannabis extracts and they work together in a synergistic manner. This is also known as the entourage effect. The high strength of all of the cannabis components provides a strong hit of medicine that gives patients quick and effective relief. \n\n### Significant Pain Relief\n\nMany patient support groups have expressed that dabs have a place in medical cannabis for patients that have extreme pain. Since dabbing provides rapid relief, it can be particularly helpful for patients who experience quick onset pain that is particularly excruciating. Concentrates are on average four times stronger than smoking a joint. This means by dabbing a patient can receive a much stronger medicinal dose in one shot. \n\nIn particular, dabbing can be helpful for patients who have problem with pain induced insomnia. Concentrates are known for their psychoactive effects because of the high levels of THC, but the high levels of THC also help patients fall asleep quickly and many of the psychoactive effects will wear off before the patient wakes up. \n\n### Extraction Methods Matter\n\nPatients should know that concentrates are concentrated forms of the plant. This means it is important to only select extracts created with organic ethanol, water, or CO2 that are grown without the use of pesticides and toxic chemicals. For this reason, the extraction methods of cannabis wax are a big source of controversy. It is important to choose cannabis extracts that are high in quality in order to not receive any adverse effects from dabbing, especially with those who may have compromised immunity.\n\nMany extracts are made with butane, which can leave a dangerous residue in the cannabis extract. CO2 extraction has been prized as the cleanest way to create a cannabis extract, but some people argue that terpenes aren’t preserved in wax created from CO2 extraction. Organic ethanol extraction, however, has been seen to have less problematic residue than butane, but preserves the terpenes in cannabis.\n\n### Strong Pain Relief\n\nThe Addictive Behaviors Journal published a study from the State University of New York in Albany which was conducted with 357 patients who participated in dabbing. All of the patients had a history of cannabis use and the study determined that dabbing causes no more problems than with traditional cannabis. \n\nThe participants reported that dabs did, however, lead to higher tolerance levels and withdrawals. The consumers within the study, overall, did view it as a more dangerous form of cannabis administration than traditional smoking. \n\nDabbing is the strongest form of cannabis that a patient can administer, but it can be helpful for a particular series of medical conditions that require a very strong dose of pain reducing medication. Dabbing is also helpful for people that have severe pain that comes on quickly and acutely, or those with pain induced insomnia. \n\nPeople considering trying concentrates for their medical conditions should be very careful when trying dabbing for the first time. With the use of cannabis in multiple forms on a daily basis, the average number of migraines decreased from 10.4 to 4.6 per month. Overall 85% of participants reported fewer migraines, 19.8% reported that cannabis helped prevent their migraines, and 11.6% of participants said that medical marijuana stopped their migraines altogether. The study also identified that the inhalation of marijuana was the best administration method for fast migraine relief. \r\n\r\n### Marijuana and Migraines: The Endocannabinoid Connection\r\n\r\nA 2008 study published in _Neuroendocrinology Letters_ found that [migraines are deeply connected with endocannabinoid function](https://www.ncbi.nlm.nih.gov/pubmed/18404144). The study stated that the possible cause of migraines could be an endocannabinoid deficiency, meaning cannabis could be extremely helpful in decreasing migraines by supplementing the body's natural store of endocannabinoids. \r\n\r\nCannabidiol (CBD) has been found to be very effective at treating migraines because of its anti-inflammatory and analgesic properties. Tetrahydrocannabinol (THC) is also helpful at relieving migraine symptoms due to its ability to greatly reduce pain. CBD-heavy strains, however, help alleviate symptoms while keeping sufferers alert. \r\n\r\nMost notably, because they have little to no psychoactivity, high CBD strains will allow a patient to get through the day without the feeling of being stoned. Most of the high CBD strains listed below do contain some THC however. Depending on how sensitive you are to THC, some marijuana strains may be borderline psychoactive. This leads to restlessness, increased stress levels and depression. \r\n\r\nAccording to the American Physical Therapy Association, depression in chronic pain patients very often leads to an increase in their originating pain and creating a pain cycle that is very difficult to get out of. While chronic pain can insight bouts of depression, depression tends to increase body stiffness, sleeplessness and stress.\r\n\r\n### Parental Hardships\r\n\r\nParenting is an athletic experience, especially for those with children under 5 years of age. Playing on the floor with them, reaching down to kiss an owie, cleaning up after play sessions. \r\n\r\nIf bending, reaching or standing for long periods of time causes chronic pain flare ups, parenting can prove to be physically and emotionally impossible. Scoliosis survivor and parent of a young daughter, Rachel Rabkin Peachman shared her experiences with The Atlantic: “My daughter was sick, and I desperately wanted to soothe her. While they are highly effective at blocking pain receptors from communicating with the brain, they also come with a slew of side effects. \r\n\r\nSide effects from dependency on pain medications can lead to a spiral of prescription drug use in order to combat breathing difficulties, insomnia, constipation and sexual side effects, just to name a few. \r\n\r\nOpioid use also has a very high risk for addiction. Over 2 million Americans are addicted to opioid pain relievers and the numbers show no signs of waning.\r\n\r\n### Financial and Economical Impact\r\n\r\nAccording to health economist studies, the US economy pours over $635 billion into chronic pain per year. This is more than cancer, heart disease and diabetes. These costs trickle down to patients, often causing disastrous consequences to household budgets and financial well-being. \r\n\r\nNationwide, the economical impact comes in many forms. Diminished productivity and the increased cost of healthcare plague businesses, especially those with physically demanding work. Chronic pain is the most common type of long-term disability in the US. The Council for Disability Awareness reported the average disability claim lasts over 2 years. For 1 in 8 workers disability claims can last over 5 years and may even be indefinite.\r\n\r\n### Tips for Patients\r\n\r\nIf chronic pain is taking over your life it’s important to reach out for emotional and mental support. Find someone to talk to who can offer guidance on how to co-exist with pain. Migraines are the third-most prevalent illness in the world, and they’re three times more likely to affect woman then men. Migraines are not only painful, but can be extremely debilitating. Ninety percent of migraine sufferers can’t work or complete daily activities while suffering from a migraine. Migraines can last up to 72 hours, greatly affecting the life of those who suffer from the affliction. Marijuana can provide relief for migraine sufferers in place of more severe pharmaceuticals.\r\n\r\nHere’s what Dr. Frye has to say about migraines.\r\n\r\n### What causes migraines?\r\n\r\nIt was once believed that migraines were caused by dilated blood vessels, but it’s now thought that migraines are generated by the neurons that supply the meninges, which is the plastic wrap-like covering around the brain. There’s thought that neuronal stimulation not only causes arteries in the area to dilate, but also causes an inflammatory reaction in the meninges and in the trigeminal nerve. It may be the combination of these actions that causes the pain associated with migraines.\r\n\r\nMigraine sufferers can also experience neurogenic pain from stimuli that normally wouldn't evoke a painful sensation, such as simply touching the scalp. The role of serotonin is complex and while not completely understood, it’s thought to be a factor in migraines as well.\r\n\r\nApproximately 75% of migraine patients can identify triggers that cause the migraines to occur. Stress is the most common trigger, affecting 80% of migraine sufferers. Triggers can also be sleeping too much or not enough, going without eating, weather changes, and certain foods or odors. \r\n\r\n![](https://hellomd-production.s3.amazonaws.com/articles/577d4650d64ada0007000013/embedded_images/577ffdacf51c31000b000039/Screen_Shot_2016-07-08_at_12.22.45_PM.png) \r\n\r\n### How can marijuana help relieve migraines?\r\n\r\nRandomized clinical studies on cannabis and migraines are lacking, but there are several mechanisms to explain why it provides relief from migraine pain. Cannabis can attenuate the body’s stress response at the level of the pituitary-adrenal axis by interrupting certain biochemical changes. CBD can be especially helpful in alleviating neurogenic pain, while it and THC have potent anti-inflammatory properties.\r\n\r\nTHC also serves as an analgesic by shutting down pain signaling at the pain receptor level. THC may also contribute to migraine pain relief by raising serotonin levels, as do certain anti-depressants, such as amitriptyline. At high doses, however, THC can have the opposite effect and cause migraine symptoms to worsen.  It’s been noted that some regular cannabis users have experienced migraines after discontinuing its use, suggesting that cannabis may have the capacity to prevent migraines. \r\n\r\n### What consumption method for cannabis is best for relieving migraines?\r\n\r\nInhalation delivers medication to the nervous system within seconds and can be used when a patient feels a migraine coming on. It’s much easier to relieve migraine pain when it’s treated early. Once the inhaled dose takes effect, tincture or spray under the tongue can be an effective next step in providing longer-lasting pain relief.\r\n\r\nIt’s important to remember that with cannabis, less is more. Cannabis works best for pain at low to moderate doses. High doses can be ineffective in treating pain and can sometimes make symptoms worse.\r\n\r\nAlways start with a low dose and titrate up every five to 10 minutes with inhalation, every 20–30 minutes with spray or tincture, because by taking too much, you can miss your optimal dose. He is also a new contributor to the HelloMD blog, discussing his own experiences with cannabis.\r\n\r\n# Cannabis Solved My Back Pain...Mostly\r\n\r\nThe pain relieving properties of cannabis are remarkable and it is for this reason that I choose cannabis as my 1st line option for combatting chronic back pain. I have experimented with over-the-counter medications in the past like Advil and Tylenol and have even used the prescription medications Flexoril and Zanaflex. They all provided a degree of relief, but due to my lack of enthusiasm for swallowing candy coated pills and a few unpleasant side-effects (ohthe grogginess), I was often left with a partial solution to my back pain problems. Once I made the switch to a cannabis maintenance plan, focusing on some stellar CBD strains, I started getting some real, lasting relief from my pain! And then my muscle tissue lived happily ever after… \r\n\r\nExcept, on some sweaty nights I still found that I woke up with back pain and, much to my concern, even sometimes spinal pain! I racked my brain for the causes... was I hyper extending too much? Not enough stretching? Over time I started to notice two things: The first was that whenever my back hurt the most my lips were very dry. The second was that on my worst days I was waking up with a headache.  Quickly, the picture began to become clear. I was finding myself waking up dehydrated! \r\n\r\n# Dehydration: The Culprit in Chronic Pain\r\n\r\nQuickly, I took to the web to confirm my hunch. Article after article confirmed my hypothesis; what a relief I felt knowing that my problem had been identified. Here’s what I found: \r\n\r\n>1.\tYour back has a series of discs in between each vertebra. \r\n2.\tThese discs are the shock absorbers that cushion your bones.\r\n3.\tEach disc is mostly comprised of water.\r\n4.\tIf the disc is dehydrated, then its ability to cushion your vertebra becomes  compromised.\r\n5.\tAs a result “pain, swelling, and even ruptures” can occur.\r\n\r\nBefore this, I never realized that dehydration had such an impact on your spinal health.  I knew the traditional methods of back care, like a daily walk, plenty of stretching, and good posture, but I had not become aware of the enormous affect water had on my back.\r\n\r\n# Cannabis - Not a Silver Bullet\r\n\r\nOnce I began monitoring my water intake, especially making sure that I was hydrated before bed, I found my spine was more flexible and in general healthier feeling. The whole experience was a great reminder that, although cannabis works wonders with chronic pain, you can’t just rely on a singular fix for your pain. You need to take care of your entire body. Unfortunately, this\ncouldn't be further from the truth. While opioids are effective at\nknocking out pain, they're also some of the most highly addictive and\ndeadly medications currently available to American consumers. In fact,\nopioid medications are so dangerous that they accounted for upwards of\n16,000 deaths in 2009 alone. Because of these horrifying outcomes and\nthe ever-increasing rates of opioid prescriptions, American health\nprofessionals and consumers alike are on the lookout for safer, less\naddictive alternatives.\n\nThe Opioid Addiction Problem\n----------------------------\n\nRecently, Castlight Health released a report titled \"[The Opioid Crisis\nin America's\nWorkforce](http://www.castlighthealth.com/press-releases/new-study-reveals-32-percent-of-total-opioid-prescriptions-are-being-abused/)\"\nthat reveals some startling new insights into the scope of the opioid\naddiction problem. Using aggregated data from 1 million anonymized\nAmericans, the researchers found that 32 percent of all opioid\nprescriptions in America are being abused. The report also found that\n4.5 percent of people who have a current opioid prescription are abusing\nit, and that these people cost American employers an average as twice as\nmuch (approximately \\$19,450 annually) in medical expense costs as their\nopioid-free counterparts. In addition, baby boomers are nearly four\ntimes as likely to suffer from opioid abuse and addiction as\nmillennials, and patients who live in lower-income areas are twice as\nlikely to suffer from opioid abuse as individuals who live in\nhigher-income areas.\n\nWhile the results of the Castlight report are startling, they're not\nnews to health professionals. The opioid crisis has been ballooning for\nyears, and it has recently gotten so bad that the CDC has declared it a\n[public health crisis](http://www.ncbi.nlm.nih.gov/pubmed/25581144).\nBecause of the dangerous side effects and highly addictive nature of\nopioid medications, health officials and consumers everywhere are\nactively searching for better alternatives.\n\nCannabis: A Better Way\n----------------------\n\nIn an attempt to mitigate the dangerous effects of opioid\nover-prescription, abuse and addiction, many people are turning to\ncannabis. In addition to the fact that cannabis has a long, demonstrable\nhistory of being used to treat chronic\npain,\ndozens of studies (such as this [2012\nstudy](http://www.ncbi.nlm.nih.gov/pubmed/22880540) published in the\nJournal of Psychoactive Drugs) have found that cannabis offers fewer\nside effects than opioids and provides more therapeutic benefits than\nsynthetic medication. Recently, prominent politicians such as Elizabeth\nWarren have been [petitioning the\nCDC](http://www.theguardian.com/us-news/2016/feb/12/elizabeth-warren-medical-marijuana-painkiller-opioid-abuse)\nto consider medical cannabis as a supplement to or alternative for\nopioid pain medications, and states like Massachusetts have been using\ncannabis to treat patients suffering from chronic pain and opioid\naddiction. Recently, one clinic in Massachusetts [used medical\ncannabis](http://www.bostonherald.com/news_opinion/local_coverage/2015/10/doctors_pioneer_pot_as_an_opioid_substitute)\nto treat 80 chronic pain sufferers who were addicted to opioid\nmedication. Within a month of treatment, 75 percent of the patients had\ngiven up their opioid medication completely and the vast majority\nreported less pain. Several years ago I was the program director for a San Francisco human and social services nonprofit, doing a job that I absolutely loved, when an injury caused systemic nerve damage and prevented me from being able to sit and use the computer for the long hours required, as well as other routine daily activities. The two years that followed were some of the harder times of my life, while I struggled to find a new way of caring for my body--one that would allow me to be pain free. \r\n\r\nLiving with chronic pain required me to do a lot of research and learning, and a lot of experimentation. I quickly realized that the side effects from pharmaceuticals were best avoided and thus began my trial and error with alternative forms of medicine. I always knew smoking cannabis could alleviate pain, but I’ve since learned that there is so much more to using cannabis than simply picking up a joint. Ultimately, cannabis and a solid self-care routine is what brought me back to health. \r\n\r\n**Beyond the Smoke**\r\n\r\nAt first I turned to smoking flower for pain relief. It did wonders--especially now that there is so much information available on strain types. The only downside of the some of the flower was I felt too high. I wanted to get back to life as usual and although I’ve always enjoyed smoking a joint from time to time, I’ve never been an all day every day smoker. I needed to find a way to manage my pain and stay more clear-minded. \r\n\r\nAs the medical cannabis industry has grown, we’ve seen an explosion in other methods of consumption. Learning about these products gave me a vast toolkit. Edibles and tinctures offer a discreet, easy to measure dose of cannabis available in many configurations. Some are low-dose, some are for sleep, some are high-CBD, and the list goes on. These products have been life-savers. But by far one class of products has helped me day in and day out more than any other is topicals. \r\n\r\n**Topicals - What Are They?**\r\n\r\nTopicals are a way to apply cannabis directly to your skin. Formulas come in many forms including oils, balms, salves, and lotion bars. Topicals are not psychoactive so you will not feel high after using them. \r\n\r\n**How Do They Work?** \r\n\r\nApplying a topical is a transdermal method of ingesting cannabis - meaning it goes through your skin. They work by binding to a network of receptors called CB2 that are found throughout the body and are activated either by the body’s naturally-occurring endocannabinoids or by cannabis compounds known as phytocannabinoids. \r\n\r\nWhen you apply most topicals, the cannabinoids don’t enter the bloodstream so you will not have a psychoactive experience. The two exceptions are with transdermal patches, which will deliver THC to the bloodstream and can cause psychoactive effects and cannabis-infused soaks, which allow THC to be absorbed through the colon. However, with lotions, salves, and the like, you can use them to relieve pain in a specific area while going about your day. \r\n\r\n**Penetration vs. Absorption** \r\n\r\nIn skincare, there is a difference between penetration and absorption. Skin penetration refers to the amount of a topically applied compound that exists between the top layer of skin, the stratum corneum, and the bottom layer of skin, the stratum basale. During penetration, the body does not absorb the compound so it does not affect the body systems. Skin absorption refers to the process where the compound breaks the skin barrier and enters the bloodstream.  \r\n\r\nMany factors influence penetration and absorption, including how thick the skin is in the area a formula is applied and the type of formula being applied. Most topicals will not pass the skin barrier and absorb into the bloodstream, but patches, for example, have been made with molecules small enough to do so. \r\n\r\n**Body Oils, Lotions and Salves**\r\n\r\nThrough my experimentation, I found each of these formulas have their own purposes. \r\n\r\nBody Oils - Coupling cannabis with self-massage (or massage from someone else if you’re lucky) can dramatically increase the therapeutic effects. Body oils stay on the skin long enough to provide lubrication for deeper massage, releasing of knots and tension. \r\n\r\nSalves and Balms - These absorb into the skin a bit faster and they are less messy to use, making them my go to for portable relief. Keeping a jar in my purse to apply as needed has been a lifesaver on many occasions. Salves and balms provide enough oil for some massage, but will sink into your skin faster than oil. \r\n\r\nBody Butters - Butters are one step down on the oily scale from salves and balms. Another great choice for relief on the go, body butters made with a natural butter base such as mango, shea, or cocoa. In my experience, the natural butters carry enough scent that they are the predominant smell, not the cannabis. \r\n\r\nLotions - the lightest in texture and oiliness, these will penetrate your skin the fastest. They are not good for massage, but they are great if you want to apply something under your clothes that will leave no mess. \r\n\r\nLotion Bars - lotion bars are solid bars that melt when applied to skin. Often made from a combination of natural butters and oils, these are great for an at home massage treatment. \r\n\r\nSoaks - A newer trend on the cannabis market are bath salts and bath bombs. Fill your tub, add the product, and soak. While you’re relaxing in the tub, the cannabis is penetrating your outer skin layers while also being absorbed by your colon. This leaves you with a very light psychoactive experience that is somewhere between very calm and slightly high. \r\n\r\n**...That Smells Nice**\r\n\r\nSome topicals have stronger cannabis scents than others. There are several options that do not smell like cannabis at all, offering you relief in places where you’d prefer not to unleash the smell of cannabis. Some companies add additional ingredients, such as wintergreen, eucalyptus, and lavender add to the experience. Some topicals are heating and some are cooling. Try a few to find your favorites. \r\n\r\n**What Can I Use Them For?** \r\n\r\nFor Inflammation and Arthritis - CBD has anti-inflammatory properties. If you’d like to combat inflammation or pain related to arthritis, look for a high CBD topical. \r\n\r\nFor Cramps - Topicals calm menstrual cramps. I’ve found body oils to be very effective, especially if applied to both the lower back and lower abdomen and paired with a hot water bottle. \r\n\r\nFor Sore Muscles - Soaking after a workout or self-massage with an oil will leave you less sore the next day. \r\n\r\nFor Better Sleep - a thorough rub down before bed with any topical makes me sleep more  soundly. My body relaxes more deeply as I drift off into my dreams. \r\n\r\n**A Self-Care Routine**\r\n\r\nFinding a way to handle pain that didn’t make me high was a game-changer. But cannabis alone is not a magic bullet. Developing a self-care routine was invaluable to my recovery. For some techniques, such as massage, pairing with cannabis magnified the healing effects.  \r\n\r\nStretching - Developing a 10-15 minute routine at the beginning and end of your day can make a world of difference. Luckily, the internet abounds with yoga and stretching videos if you’d like inspiration. \r\n\r\nMovement - A vital part of each day, especially if your job requires you to sit in one place. A brief daily walk or getting up once per hour move a bit will make the world of difference. Topical balm can be used in advance of any activity to loosen muscles, as well as be applied afterwards to soothe soreness.\r\n\r\nMassage and other Body Work - Massage alone is magnificently healing. Pairing it with a cannabis brings another level of relaxation. While a professional body worker will reach very deep levels of healing, self-massage on a regular basis is also fantastic. \r\n\r\nMini Foot Spa - While integrating massage into your life is a worthwhile luxury due to the health benefits, it’s not always possible to get a massage. Doing your own mini foot spa is another at-home, accessible self-care routine. Our feet carry so much tension a foot spa can release. Try a cannabis soak and rub some bath salts into your feet to exfoliate. Follow with an oil applied with massage. If you want to take it to another level, look at a reflexology chart and target areas specific to your tension. \r\n\r\nTake A Minute to Breathe - Much like stretching, just a few minutes each day makes a world of difference. While I was initially skeptical, meditation taught me a way to stay grounded during a flare up. I developed a pre-meditation ritual of rubbing some of my favorite scented topical into my temples and neck beforehand, helping me to settle in. \r\n\r\nAs I’ve come to find is often the case for providers of cannabis medicine, it was my own discovery of the vast and varied healing properties of this plant that inspired me to share it with others. According to the Centers for Disease Control and Prevention (CDC), up to [25% of people](https://www.cdc.gov/drugoverdose/data/overdose.html) who receive prescription opioids for non-cancer-related primary care become addicted to the substance, with drugs like methadone, oxycodone and hydrocodone being the most commonly abused.\r\n\r\nThe statistics are alarming and frightening. Finally, the medical industry is taking steps to curb the opioid addiction epidemic. The CDC is urging doctors and health professionals to be very cautious when prescribing painkillers and recommends they avoid it altogether for certain patients’ chronic pain treatment.\r\n\r\nFOLLOW US ON [FACEBOOK](https://www.facebook.com/pg/hellomdcompany/posts/) & [INSTAGRAM](https://www.instagram.com/hellomd_com/?hl=en) \r\n\r\n### What Does the New Opioids Guidance Mean for Patients?\r\n\r\nWith the exception of those receiving end-of-life treatment or cancer care, doctors are now advised to, if possible, simply not prescribe opiates to patients with chronic pain or those suffering from injuries. Although the CDC recognizes that there are some situations in which the benefits of opioids outweigh the risks, current [CDC guidelines](https://www.cdc.gov/drugoverdose/pdf/guideline_infographic-a.pdf) recommend that doctors try other drugs, such as anti-inflammatories, as first-line treatments before prescribing opiates for pain relief. The CDC also recommends starting with a low dose of opioids and building up to a dose that’s effective at relieving pain, rather than immediately prescribing a high dose.\r\n\r\nMany in the medical industry believe that the risk of opioid addiction outweighs the benefit of temporary pain relief. After all, the numbers don't lie; painkillers are deadly and are currently abused by [nearly two million people](https://www.health.harvard.edu/newsletter_article/painkillers-fuel-growth-in-drug-addiction). Why run an ad if if they couldn’t make money. So of course they have a drug to treat this! Of course the company that ran the ad, Astra Zeneca, dreamed up an oh-so-cute name for their drug. Movantic (get it – mov- like move your bowels) costs about $300 per month. There even is another drug by Salix named Relistor ( relis – like release your bowels I guess) is actually a injection you give yourself and goes for about $400. What a deal.\n\nHere’s a suggestion. STOP TAKING OPIOIDS! There are about 38 million people who are prescribed opioids each year, with some people having monthly refills, resulting in 259 million prescriptions written in 2012. Between 40-80% of these people suffer from opioid induced constipation. So you can imagine the incentive for Big Pharma in creating a drug to treat a side effect of a drug. They benefit from both ends. What does this result in? \n\nHow about 16,000 people dying per year from overdose of opioids. Over 420,000 emergency department visits relating to abuse of narcotic pain relievers. DO you know how to stop this from happening? STOP TAKING OPIOIDS!\n\nThere is a solution.
