After suffering for years from chronic pain due to intense migraines and fibromyalgia, [cannabis allowed me to stop using narcotics](https://www.hellomd.com/articles/how-we-can-disrupt-the-opioid-epidemic-with-cannabis). Those traditional pharmaceuticals didn’t provide much benefit other than occasionally masking the pain. Once I turned to cannabis, I found a solution that not only prevented my chronic pain but also helped me manage pain when it crawled through. Once I realized cannabis was a possible solution for me, I began experimenting and eventually found that microdosing was my path to success.\n\n![Hello6-min (6).png](https://hellomd-cms.s3.us-west-2.amazonaws.com/Hello6_min_6_6f0daaad5e.png)\n\n### Microdosing Cannabis as Effective Pain Relief\n\nWhen many people talk about microdosing, they mean taking small amounts of cannabis, waiting to feel the effects, and then taking more until you reach your desired result. The microdosing I want to talk about is taking small amounts of cannabis on a consistent basis to receive health benefits without the psychoactive effect.\n\nRespected cannabis doctor [Dr. Dustin Sulak noted substantial anecdotal evidence](https://www.projectcbd.org/sobre-cbd/clinical-research/dr-dustin-sulak-cannabis-dosing) within his practice over a period of years with microdosing. He said in a recent article, “*I discovered that most people have a certain threshold dosage of cannabis, below which they’ll actually experience a gradual increase in health benefits over time, and above which they’ll start building tolerance, experiencing diminishing benefits, and more side effects.*”\n\nAs with so much within the cannabis community, we depend upon anecdotal evidence as clinical research is scarce due to marijuana’s classification as a [Schedule 1 drug](https://www.dea.gov/drug-information/drug-scheduling#:~:text=Schedule%20I%20drugs%2C%20substances%2C%20or,). However, a recent [study conducted by University of California, San Diego, demonstrated positive results with microdosing](http://www.ncbi.nlm.nih.gov/pmc/articles/PMC3566631/). The study focused on subjects suffering from neuropathic pain and had them microdose through vaping. The results indicated that the patients who microdosed cannabis at 1.29% felt as much pain relief as the patients who used a medium dose at 3.53% cannabis. Those who microdosed less also felt better for a longer period of time than participants who ingested more.\n\n![Hello1.png](https://hellomd-cms.s3.us-west-2.amazonaws.com/Hello1_63481b85e9.png)\n\n### Lower Doses of THC May Enhance the Endocannabinoid System\n\nWhat’s most interesting about this study is its contrast with the average amount of [tetrahydrocannabinol (THC)](https://www.hellomd.com/tag/thc) found in most vape pens or joints, which often stand at 14% and may even go as high as 95%. The amount of THC used within microdosing is substantially lower than what most consumers ingest in a typical sitting. Some hypothesize that microdosing on a consistent basis results in more positive benefits than higher doses because lower doses of cannabis enhance the [endocannabinoid system](https://www.hellomd.com/articles/what-is-the-endocannabinoid-system) while much larger doses make our CB1 and CB2 receptors—the receptors that control appetite, pain, mood, and memoryless responsive. \n\n![Hello3.png](https://hellomd-cms.s3.us-west-2.amazonaws.com/Hello3_de8707650f.png)\n\n### My Personal CBD Microdosing Regimen\n\nMy version of microdosing includes a daily dose of tinctures high in [cannabidiol (CBD)](https://www.hellomd.com/tag/cbd); I like a ratio of 20:1 CBD to THC. I also use a low-dose tincture with 2.5 mg of THC per dose as many as two to three times a day. \n\nSince these THC doses are incredibly low, I never feel high, if anything, I just feel mildly relaxed. When a migraine does manage to sneak through, which is now rare, I vape a high-CBD pen of any one of the strains [Canna Tsu](https://www.hellomd.com/strains/canna-tsu), [ACDC](https://www.hellomd.com/strains/acdc), or [Harlequin](https://www.hellomd.com/tag/harlequin/). These strains contain around 20% CBD to 5% THC. Again, my goal is to manage pain and not get high, and without a doubt, I reach my goal. \n\nMicrodosing allows me to live a very full life, prevent chronic conditions, and manage pain when it happens. Without cannabis and this regime, I would be in bed for multiple days a month. These new consumers are faced with a confusing and disaggregated system. When walking into a typical dispensary, you’ll often come face to face with a 20-something budtender who might suggest an Indica gummy bear that contains 100 mg of THC, a chocolate bar with 1,000 mg, or a vape oil cartridge with a whopping 95% THC. What we need is a plethora of products that are between 2.5-5 mg per dose, whether that be a tincture, edibles, sublingual spray, or high CBD/low THC vape oil cartridges. \n\nThe bottom line is that the market has yet to catch up with consumer trends. After initially rating their pain from zero to 100, the study participants were divided into groups, with some taking cannabis oil containing tetrahydrocannabinol (THC) and others taking a placebo. \n\nThe process was repeated a week later, with participants who received the cannabis oil now taking the placebo. What the study found is that patients taking the cannabis oil reported a significant lessening of their pain.\n\n\"Our study results link pain relief from THC with a reduction in the connections between areas of the brain otherwise heavily connected, suggesting that THC may alleviate pain by disrupting signals between these pain processing pathways,\" said Dr. Haggai Sharon, one of the study’s authors.\n\nAnother study, conducted in October 2018 by Canada’s McGill University Health Centre found that [low doses of cannabidiol (CBD) given to animals not only alleviate pain, but also reduce anxiety](https://www.sciencedaily.com/releases/2018/10/181024163625.htm). \n\nAnd a third report published by an ad hoc committee of the National Academies of Sciences, Engineering, and Medicine found there was “[conclusive or substantial evidence that cannabis or cannabinoids are effective in the treatment of chronic pain in adults](http://nationalacademies.org/hmd/~/media/Files/Report%20Files/2017/Cannabis-Health-Effects/Cannabis-conclusions.pdf).” \n\nThe authors of the report compiled over 10,000 medical cannabis studies undertaken since 1999, and though not all of their findings pointed to cannabis as a global cure-all, its use for pain relief was found to be definitive.\n\n### Not Every Study Confirms Marijuana’s Pain Reduction Properties \n\nTo be fair, other studies haven’t shown conclusive evidence that cannabis eases pain. In some cases, the number of people in research groups was too small. I have to admit that as much as I’ve learned about the therapeutic uses of marijuana, I still have one foot in the “cannabis is an effective medicine” camp and the other in the “oh, no, I can’t use cannabis daily and still function” camp. \n\nWanting to learn more about daily marijuana use—and hopefully overcome some of my own nervousness around it—I turned to Dr. Perry Solomon, chief medical officer at HelloMD.\n\nFOLLOW US ON [FACEBOOK](https://www.facebook.com/pg/hellomdcompany/posts/) & [INSTAGRAM](https://www.instagram.com/hellomd_com/?hl=en)\n\nAccording to Dr. Solomon, our body’s endocannabinoid system (ECS) can become deficient, and consuming tetrahydrocannabinol (THC) and cannabidiol (CBD)—two of the main cannabinoids in cannabis—can help our bodies restore an internal balance or homeostasis.\n\nOccasional cannabis consumption may not deliver the anti-inflammatory, anti-anxiety and relaxing feeling that you can get from THC and CBD. This is why daily marijuana consumption can be effective as part of a health maintenance and overall wellness regimen.\n\n### The Impact of Daily Cannabis & CBD Use on Our Health & Wellness \n\nI asked some women who incorporate cannabis or CBD into their daily routines why they do it. Each one of them initially turned to—or returned to—cannabis to address very specific and often debilitating health issues. Each found their daily marijuana consumption habits brought additional health benefits.\n\nMarcy began taking CBD for spinal pain and inflammation after an injury. She was able to get off four pharmaceuticals. And she also reduced her pain from an 8 to a 3 on the pain scale. \n\n“High-CBD strains have changed my life. I rarely even have to take Advil or Aleve,” Marcy says. “While I wasn’t taking CBD for mental health issues, I found myself able to get off all medications for depression and anxiety but one: an emergency medication in my purse for panic attacks.”\n\nMarcy says she’s also interested in the neuroprotective qualities and bone-building effects of CBD when taken daily. She consumes a 25 mg high CBD/low THC capsule twice a day, once in the morning and once in the evening. She prefers ingesting cannabis rather than smoking or vaping the plant. \n\nDr. Solomon recommends taking cannabis in tincture, edible, oil or suppository form over smoking or vaping cannabis. \n\n“The irritation to the lungs of the two latter methods can be bothersome and cause coughing as well introduce the possibility of inhaling particulate matter,” he says.\n\nDr. Solomon says that microdosing—taking smaller amounts of cannabis over time—is a good way to get the daily health benefits of the plant without psychoactive effects. People typically microdose 2.5 mg of THC at a time. \n\nBut for some people like Marcy, 25 mg of a CBD-rich cannabis product is a microdose. Each person’s microdose can differ based on myriad factors including the form of consumption and even a person’s metabolism.\n\nAnother woman who turned to daily cannabis to deal with pain is Josie, who was able to get off the morphine she was taking for pain. Stricken with rheumatoid arthritis since she was 14, another part of her treatment for the condition is chemotherapy treatments twice a year to suppress her immune system. Side effects of this treatment include intense bone pain and a suppressed appetite that leads her to being underweight. \n\nFor Josie, daily cannabis consists of a strong indica strain that reduces her pain and increases her appetite. She finds that her daily cannabis use also reduces her anxiety. \n\nTasha is yet another woman I spoke to who also consumes marijuana regularly to reduce anxiety by either smoking, vaping or ingesting—but almost always at night. She finds it greatly reduces her anxiety, and she’s also experienced other benefits. \n\n\"I was diagnosed with endometriosis almost 20 years ago and a result of that is inflammation flare-ups that my own body is creating,\" Tasha explains. \"I also had sinus surgery that involved all of my face a few years ago as a result of polyps. After extensive allergy testing, no one could give me an answer as to why polyps were developing other than inflammation my body was generating. They gave me a 60% chance the polyps would grow back after the surgery.\"\n\nTasha was excited about the anti-inflammatory properties of cannabis. Her sinus check-up last year showed no signs of the polyps returning. She is also starting to experiment with adding cannabis strains high in CBD to aid in recovery when weightlifting three times a week.\n\n### What to Know When Consuming Cannabis or CBD Daily \n\nIf you’re thinking about taking cannabis or CBD regularly, Dr. Solomon advises that you consider the following:\n\n* **Be aware of the concentration of THC or CBD** you’re taking. Dosing is an individual process, so start at a low dose and slowly increase the amount.\n\n* **Look up the laboratory testing results** of what you’re buying. The product you purchase should have a label of components and be tested for purity as well as things like pesticides, bacteria and mold. \n\n* **Try to find out the source of the hemp or cannabis** if you can. Knowing where the product is coming from gives you a better idea of how it was produced and what you’re consuming.\n\nDon’t want to feel high on THC, but still want to reap the health and wellness benefits of the cannabis plant? Dr. Solomon says that he’s seeing more and more women and men who don’t want the psychoactivity, and are therefore turning to CBD products.\n\n“The exact amount of CBD that should be ingested can vary from 25 mg several times a day to several hundred milligrams depending on what you’re looking for,” he says, adding, “It can sometimes take several weeks to feel the effect as you slowly increase your dosage.”\n\nYou don’t need to have an acute or chronic health issue to explore the benefits of cannabis or CBD. This repeated use can cause the affected area to swell and become inflamed, thereby compressing or pinching nerves. Three of the most common vaginal infections are: \n\n* **Bacterial vaginosis:** A very common infection caused by an imbalance or overgrowth of a bacteria called Gardnerella. Gardnerella is actually a naturally present bacterium in the vagina. But when it grows beyond its normal level, it can cause some serious discomfort like:\n\n>* Vaginal discharge\n* Odor\n* Itching\n\n* **Yeast infection:**  Yeast-based infections, also known as candidiasis, are the most common vaginal infections. Having a normal level of candida, the fungus associated with yeast infections, is nothing to worry about. Natural bacteria in your private parts will usually keep the growth of yeast under control. It’s only when the levels of yeast rise to a higher concentration that it can cause issues. These issues can include:\n\n>* Inflammation\n* Odor\n* Irritation\n* Itching\n* High levels of milky discharge\n\n* **Trichomoniasis:** Trichomoniasis is the only sexually transmitted infection on this list. It also happens to be the most commonly contracted curable sexually transmitted disease, [affecting 3.7 million Americans](https://www.cdc.gov/std/trichomonas/stdfact-trichomoniasis.htm). Trichomoniasis is caused by a microscopic parasite found in the vagina and urethral tissues. While it almost always affects women, it’s possible for men to get the infection as well. \n\nRELATED: [HOW THE ENDOCANNABINOID SYSTEM AFFECTS WOMEN’S HEALTH](https://www.hellomd.com/health-wellness/5a271593100a820006562eb8/how-the-endocannabinoid-system-affects-womens-health) \n\n### Marijuana’s Antibacterial & Antifungal Properties Can Help Treat Vaginal Infections\n\nAll of the vaginal infections we’ve talked about come from the overgrowth or appearance of bacteria or fungus. Interestingly, certain cannabinoids, the natural chemical compounds found in cannabis, have shown antibacterial and antifungal properties. These properties could prove to be effective medicine for vaginal infections and their symptoms.\n\nIn one study from 2008, published in the _Journal of Natural Products_, researchers concluded that five major cannabinoids:\n\n* [Cannabidiol](https://www.hellomd.com/health-wellness/5a8dafa0f24d1300077ccb59/whats-so-great-about-cannabidiol-or-cbd) (CBD)\n* Cannabichromene (CBC)\n* Cannabigerol (CBG)\n* [Cannabinol](https://www.hellomd.com/health-wellness/5a00c7e5462abf14dd36415f/having-trouble-sleeping-try-non-psychoactive-cbn) (CBN)\n\nall [show “potent activity” against bacteria like MRSA](https://pubs.acs.org/doi/abs/10.1021/np8002673). \n\nThese results seem to, in part, back up a 1981 study from the _Journal of Clinical Pharmacology_, in which [CBC was tested as an antibacterial and antifungal on rats](https://www.ncbi.nlm.nih.gov/pubmed/7298870). The researchers concluded that CBC has strong antibacterial activity and mild to moderate antifungal properties. \n\nThe _British Journal of Pharmacology_ went a bit further with a 2011 study, explaining that while CBD, CBG and CBC are all just moderate antifungal agents, these three cannabinoids [may increase the power of caryophyllene oxide, a highly effective antifungal](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3165946/). According to the study, CBG and CBC both eradicated a common fungal infection in humans in 15 days. This is a comparable rate to many pharmaceutical antifungal drugs. \n\n### Cannabis’s Anti-Itch & Anti-inflammatory Properties Can Ease Vaginal Infection Symptoms\n\nThe power of cannabis as an anti-inflammatory is one of the most widely known medical applications of the plant. And while the body of research on cannabis as an anti-inflammatory isn’t directly connected to vaginal infections, much of the uncomfortable symptoms of vaginal infections are directly associated with inflammation. \n\nInflammation can not only cause pain, but also itching, which is a common symptom of most vaginal infections. A [study from the _Journal of the American Academy of Dermatology_ looked into the role of cannabinoids in dermatology](https://www.jaad.org/article/S0190-96221730308-0/abstract).\n\nThe lead researcher, Dr. Robert Dellavalle, noted that of all the applications for cannabinoids in dermatology, perhaps the “most promising role for cannabinoids is the treatment of itch.” Dr. Dellavalle notes that in one study, eight of 21 people who applied cannabinoid cream twice a day for three weeks saw a complete elimination of severe itching. \n\n### The One Way You Shouldn’t Use Cannabis to Treat Vaginal Infections\n\nResearch indicates [daily smoking of cannabis and/or tobacco can harm the ability of macrophages in the lungs to kill candida albicans](https://www.hellomd.com/answers/5737cde2d6b1ca000b000037/can-marijuana-help-with-yeast-infections), the fungus most commonly associated with yeast infections. \n\nIn a 2004 longitudinal cohort study by the department of obstetrics and gynecology at Case Western University, 1,248 women were observed over a 12-month period. The researchers concluded there was [an association between vaginal yeast colonization and marijuana use](https://www.pubfacts.com/detail/15516380/Vaginal-yeast-colonization-in-nonpregnant-women:-a-longitudinal-study) in the previous four months. That said, there was no indication that yeast levels were high enough to cause an infection. \n\nSo, while smoking cannabis may cause elevated levels of yeast, it has more to do with the smoking of plant matter than it does the plant matter itself. So, if you’re thinking of including marijuana in your vaginal infection treatment plan, go for a cannabis tincture or topical. \n\nA cannabis topical can be applied to the lower abdomen or back, which can sometimes feel painful and sore during a vaginal infection. Some topical marijuana products are designed specifically to be applied directly to the vagina, but with other cannabis topicals, be careful about applying them down there. The last thing you want to do is to further irritate the area. \n\nIf you want to try out a sublingual cannabis tincture, choose one that will target your needs. If you want something to reduce inflammation and itching, go for a high-CBD cannabis tincture. If you need something to dull the pain of the infection, you may want to try a product with a higher-THC content, depending on how well you tolerate the cannabinoid.  \n\nOverall, certain cannabinoids show promising antibacterial and antifungal power that could help treat vaginal infections. The immune system attacks the joints, leading to inflammation and swelling of the joint linings, also called the synovia.\n\nIf left untreated, rheumatoid arthritis can lead to severe joint damage. Like many autoimmune diseases, we aren’t sure what causes rheumatoid arthritis. However, researchers think that genetics, environmental factors and possible infectious exposures play a role. \n\n### An Altered Endocannabinoid System in Osteoarthritis & Rheumatoid Arthritis Sufferers\n\nInside all of us is a network of receptors called the [endocannabinoid system (ECS)](https://www.hellomd.com/articles/what-is-the-endocannabinoid-system). This system has receptors that are located throughout the body: in your brain, your gut, even your immune cells. Due to the prevalence of ECS receptors, scientists now think that the ECS plays a key role in essential life processes like eating, sleeping and reproducing. \n\nThe main receptors in the ECS are CB1, CB2 and TRPV1. Your body also makes endocannabinoids: anandamide (AEA) and 2-arachidonoylglycerol (2-AG), molecules that attach to these receptors to produce certain effects such as pain relief. Likewise, cannabis makes phytocannabinoids, like tetrahydrocannabinol (THC) and cannabidiol (CBD) that can also attach to, or influence, these receptors. \n\nThe ECS plays a role in our immune process and how our bodies register pain. In patients with osteoarthritis and rheumatoid arthritis, that system is altered. A study published in _Arthritis Research & Therapy_ looked at the joint fluid of both osteoarthritis and rheumatoid arthritis patients. Researchers found that both [CB1 and CB2 receptors were present in the patients’ joint linings](https://arthritis-research.biomedcentral.com/track/pdf/10.1186/ar2401). They also discovered that AEA and 2-AG were present in the joint fluid of both types of arthritis patients, and not present in the fluid of healthy volunteers. \n\nThe presence in the joint fluid of both endocannabinoid receptors and the substances that attach to them have made scientists speculate that manipulation of the ECS can lead to therapies that can address both osteoarthritis and rheumatoid arthritis. \n\n### CBD Reduces Pain in Animal Models of Osteoarthritis\n\nFor now, the only published research on osteoarthritis and cannabis has been done in animal models. \n\nOne study looked at the effect of CBD on rats with osteoarthritis. The researchers injected the cannabinoid at the arthritis site and found that [CBD was able to stop pain sensations](https://www.ncbi.nlm.nih.gov/pubmed/28885454/). They also noted that CBD prevented the development of further pain and nerve damage. \n\nAnother study looked at the effect of CBD applied transdermally on rats with osteoarthritis. Researchers found this method of administration to be effective: The rats had [reduced joint swelling and pain](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4851925/). \n\nResearch on osteoarthritis and cannabis continues, with [a human clinical trial](https://clinicaltrials.gov/ct2/show/record/NCT02324777) set to be completed in December 2018. \n\n### Targeting the CB2 Receptor May Be the Key to Alleviating Rheumatoid Arthritis\n\nThe endocannabinoid system’s deep involvement in the body’s immune response has led researchers to speculate that this system can be targeted to alleviate rheumatoid arthritis symptoms. \n\nThere’s specific interest in the CB2 receptor, which is abundant in immune cells. Activating the CB2 receptor may relieve rheumatoid arthritis symptoms by controlling the body’s inflammatory response. Targeting these receptors may have widespread applications for other diseases, including [osteoarthritis](http://journals.plos.org/plosone/article?id=10.1371/journal.pone.0080440), [endometriosis](https://www.hellomd.com/articles/how-cbd-can-help-relieve-endometriosis-pain) and [Parkinson’s disease](https://www.hellomd.com/articles/how-parkinsons-disease-affects-the-endocannabinoid-system).\n\nA 2014 study found that [humans with rheumatoid arthritis had a higher prevalence of CB2 receptors in their joint tissues than those with osteoarthritis](https://bmcmusculoskeletdisord.biomedcentral.com/articles/10.1186/1471-2474-15-275). Researchers also showed that activating this receptor leads to less joint inflammation and bone deterioration in mice models of this disease. \n\nThere’s a good chance cannabis’s components can target CB2, though more research is needed. THC sometimes binds to this receptor, though it often doesn’t. We still aren’t clear on how CBD influences CB2, but we know that the terpene [beta-caryophyllene](https://www.hellomd.com/articles/know-your-terpenes-beta-caryophyllene), which is found in cannabis and other plants, can activate this receptor. \n\n### Clinical Trial Shows Promising Results in Relieving Rheumatoid Arthritis Pain & Stiffness\n\nTo date, only one clinical trial has been conducted on the effects of cannabis-based medicines in rheumatoid arthritis. In this double-blind, randomized, placebo-controlled study funded by GW Pharmaceuticals, patients were given an oral spray of THC and CBD in a 1:1 ratio for five weeks. \n\nUltimately, the researchers found [significant pain reduction and improvement of other rheumatoid arthritis symptoms](https://academic.oup.com/rheumatology/article/45/1/50/1788693) like morning stiffness, in patients using the cannabis-based medicine. The researchers called their results “encouraging” and emphasized the need for more studies of this nature.  \n\nAs the U.S. population ages, incidences of arthritis—especially osteoarthritis—will rise. Current methods of managing arthritis have their shortcomings and in many cases only provide minimal relief.
