So, getting the right amount of marijuana for the effect you’re looking for can be tricky. But a given substance doesn’t automatically have its full effect on the body once it’s consumed. Many factors can affect how long it takes for the substance to kick in. And whether that effect is weaker or stronger than it should be—or just right.\n\nVarious factors can affect the bioavailability of a substance, including cannabis, such as a person’s:\n\n* Age\n* Weight\n* Overall health \n\nThen there are circumstantial factors, such as whether:\n\n* The substance is taken with or without food\n* It’s consumed early or late in the day\n* The person is also taking compatible or incompatible foods and drugs\n\nOne of the most important factors affecting a substance’s bioavailability though, is the way it’s consumed. But they take different routes to get there, and this can affect how much THC or CBD becomes available for use—and how quickly the effects can be felt. \n\n### Variations in Bioavailability Can Make Accurate Cannabis Dosing Difficult\n\nBecause so many factors can affect the bioavailability of cannabinoids, it can be hard to get the right amount for your purposes. For example, [it’s important to consume some kind of fat, such as butter or coconut oil](https://www.hellomd.com/health-wellness/5b4fbaf58904b90007731b33/why-you-need-to-eat-fat-with-your-cannabis), with cannabis edibles, because cannabis is fat soluble. This means fat molecules can better transport cannabinoids into the bloodstream. \n\nWithout fat, the body isn’t able to fully take up the cannabis, so the plant’s effects could be much weaker than if it had been eaten with a bit of fat or oil. And if a person who usually vapes cannabis for a medical condition switches to edibles, they may be frustrated by the lag time in feeling its effects and be tempted to take more.\n\nGiven the many different cannabis strains and their varying potency, and the multitude of other variables in each person’s life, it can be difficult to calculate the right dose of cannabis for a particular health condition. \n\nIn general, however, knowing a little about how cannabis is processed and used by the body can help you decide what form—and what amount—can work best for you. \n\nHere’s a rundown of the many methods of using cannabis and what’s known about the bioavailability of each.\n\n### Smoking & Vaping Marijuana Bring Cannabinoids Directly Into the Lungs & Then the Bloodstream\n\nSmoking and vaping cannabis are among the fastest ways to feel the effects of THC and CBD. This is because inhaling cannabis brings cannabinoid molecules directly into the small capillaries in the lungs, and from there directly into the bloodstream. Once in the bloodstream, they can travel directly to the brain and other parts of the body that contain cannabinoid receptors for an effect that can be felt within minutes.\n\nThe kind of cannabis strain or the way it’s delivered can also influence the effects of inhaled cannabis. Smoking involves combustion—burning the leaves and buds of the cannabis plant itself. But although vaping and dabbing avoid the toxins produced by combustion, they involve more concentrated forms of cannabis such as oils and dabs.\n\nIn general, inhaled cannabis has a typical bioavailability of up to 56%. What this means is that 56% of the active ingredients become available to the body.\n\n### Cannabinoids From Marijuana Edibles Don’t Make it to the Bloodstream\n\nCannabis edibles are more popular than ever, and new infused foods are appearing on the market all the time. But while getting your cannabis with candies or cookies can be tasty and fun, this can be among the least effective ways to absorb a significant amount of cannabinoids.\n\nThat’s because of a phenomenon called the first-pass metabolism. Before cannabinoids can get into the bloodstream, they must take a “first pass” through the digestive system, which strips out many of the available cannabinoids and accounts for the lag time in feeling their effects. \n\nLike many other substances, the liver processes cannabinoids, where [a select set of enzymes converts them to a usable form](https://www.projectcbd.org/science/cannabis-pharmacology/cbd-drug-interactions-role-cytochrome-p450). From there, fat molecules can help transport the cannabinoids into the bloodstream and on to the brain and other tissues.\n\nBut a large percentage of the cannabis in an edible is destroyed in the journey, or simply excreted. This makes the average bioavailability of marijuana edibles hover between 4–12%. \n\nRELATED: [COOKING WITH CANNABIS](https://www.hellomd.com/health-wellness/56f9cbe5eb647f000800007f/cooking-with-cannabis)\n\n### Marijuana Sublinguals, Lozenges & Drinks Take Effect Quickly via the Mouth’s Mucous Membranes\n\nAlthough sublingual drops and tinctures, lozenges and cannabis-infused drinks could technically be considered edibles, these forms of cannabis are somewhat different. They take a very different—and much faster—route for delivering cannabinoids to the bloodstream: the mucous membranes of the mouth.\n\nBecause areas such as the base of the tongue, the cheek and the roof of the mouth are highly permeable, cannabis products that come directly in contact with them don’t have to pass through the digestive system and liver in order to become bioavailable. \n\nThe most permeable area of the mouth is under the tongue, which is why sublingual cannabis preparations are among the fastest and most effective ways to get a consistent dose of cannabinoids. After that, lozenges held against the cheek or roof of the mouth can also deliver cannabis quickly and consistently. \n\nOverall, oral cannabis preparations have a typical bioavailability of 50–75%.\n\n### Cannabis Transdermal Patches Provide Long-Lasting Relief\n\nTransdermal patches can deliver consistent therapeutic doses of cannabinoids while avoiding the first pass slowdown of the digestive system. Though there isn’t much research on the bioavailability of transdermals, patches can deliver a consistent dose of cannabinoids for long periods of time, usually between eight and 12 hours.\n\nUnlike many other kinds of medications, cannabis comes in a variety of forms, each with its own way of providing its effects. The product line, which includes six different products, has individual cannabinoids for each formulation. So, you don't get a mixture of cannabinoids; you're getting a single cannabinoid that you can use. And you can mix and match with the others.\n\nFor instance, Remedy is our cannabidiol (CBD) tablingual, which eases mental and physical stress. And Relieve is our tetrahydrocannabinolic acid (THCA) tablingual, designed to alleviate pain. We often suggest taking them together.\n\nOur tablinguals are based on single cannabinoids:\n\n* **Elevate tablingual**, which contains tetrahydrocannabinol (THC), is meant to give a classic high, uplifting both mind and body.\n\n* **Relieve tablingual**, with THCA, doesn’t produce any high and is designed to alleviate pain.\n\n* **Soothe tablingual* contains Delta-8 THC; it gives a mild high that relaxes both mind and body.\n\n* **Remedy tablingual**, with CBD, creates a barely there high, easing mental and physical stress.\n\n* **Calm tablingual** contains cannabigerol (CBG), which can help ease mental and physical stress without any accompanying high.\n\n* **Stimulate tablingual**, which has tetrahydrocannabivarin (THCV), is meant to give an energizing high and focuses the mind.\n\n### Why is it important to allow people to experiment with individual cannabinoids such as CBD and THCA? \n\n**CE:** We feel it’s important for people to experiment and understand which products and in what combinations work best for them. We all have our own individual endocannabinoid system, or ECS. This leads people to have unique experiences based on their own body composition. For instance, some people like indicas and other people may like sativas, but it's important for people on their cannabis journey to have a tool kit so they can discover what's going to work best for them. \n\nRELATED: [WHAT IS THE ENDOCANNABINOID SYSTEM?](https://hellomd.com/articles/what-is-the-endocannabinoid-system)\n\n### In your experience, do most people do well with a single cannabinoid, such as the Calm CBG tablingual, or is the power in the combination of the cannabinoids?\n\n**CE:** I think it's yes to both of those. Most people can find a single cannabinoid that does work well for them. But when you start to combine cannabinoids, that's where the synergy, the real magic, of cannabis comes together. \n\nCannabis is about polypharmacy. So it's the combination entourage effect, if you will, of different components working in concert; they give you the maximum effect so you can have an effective treatment with a single cannabinoid. But really when you start combining them, that's when you can start getting the most therapeutic benefit from cannabis. \n\n### You’ve actually created a whole new cannabis product type here. What makes the tablingual different from other marijuana products? \n\n**CE:** So, when you ingest cannabis from an extract, such as an oil, you’re taking something that's essentially an oil, and you're putting it into a water environment: your mouth. We know that oil and water don't mix. At Level, we've engineered a tablet that allows you to sublingually take a very small amount of active cannabinoid, which is taken up through your mucous membranes in your mouth and enters into your bloodstream very quickly. The onset is pretty fast, at about five to 10 minutes. It wasn’t an easy thing to achieve; it was actually quite challenging. \n\n### Would you say a tablingual or sublingual is more effective, when microdosing cannabis, than a marijuana edible that's being metabolized by the liver?\n\nIf you were to swallow an oral tablet that’s 3 mg, it takes a lot longer to get into the system, and the potential for the cannabinoids to be spread out or last within the body is much greater. When you eat an edible and swallow it, you’re putting it through an acidic stomach environment, which then has to be absorbed through the intestines and go to the liver. There are a lot of different biological processes that can happen during that time. \n\nWhen you take a tablingual, it's immediately taken up into your mucous membranes or any sublingual route of administration. And then it goes through a different pathway; it actually goes through one of the major veins that takes it right into your heart and is then distributed throughout your system. Is this meant to show people how to take tablinguals in combination?\n\n`embed: https://www.instagram.com/p/BjlCne4BAp6/` \n\n**CE:** Yes. When people enter into cannabis consumption slowly, and they don't have any adverse events and they're not scared away, they build a lot more confidence. They can trust themselves and their own judgment. At this point, they may be willing to experiment more. Right now, we're in a place where there’s a lot of experimentation. The wheel was developed for people who have the confidence to try combining cannabinoids, and we try to give some guidance in how we might do it.\n\nFor instance, pain is one of the most common conditions. When you look at the wheel, there are many different cannabinoid combinations. So. for instance, for pain, the first thing we recommend is a THCA and CBD together. There's no psychoactivity associated with that combination, and the suggestion combines two very powerful anti-inflammatory cannabinoids together.\n\nIf someone is dealing with inflammatory pain, you actually need THC in the system as well, so adding a THC tablingual may be the right thing for that particular person. [Check out our “How to Microdose With Cannabis” event] scheduled for Thursday, July 12.\n\n### What components of cannabis or what products would people use to treat their insomnia? \n\n**Chris Emerson:** Yeah, I think a lot of times it's been about products with delta-9 tetrahydrocannabinol (THC). After you use THC, you don’t feel tired necessarily, but it's not an energizing thing for the most part. By the time you get done with THC, you do feel a little sluggish. So, as people use delta-9 THC, I think it can help facilitate sleep.\n\nThere are a lot of different reasons why some of the other cannabinoids that are coming on the market, especially cannabidiol (CBD) and cannabinol (CBN), that may not have—for most people—the psychoactivity of delta-9 THC, can also be really good for helping people relax, and to try and treat persistent sleep issues.\n\n### Some people have a problem going to sleep, and some people have a problem staying asleep. Would there be different ways to treat each of those types of insomnia? \n\n**CE:** Yeah, definitely. I think so. Some people just need help relaxing to fall asleep. Once they fall asleep, they can stay asleep; that's one application. Some people are going to fall asleep, and then they'll rebound and wake up at two or three in the morning—mind's racing, the body's tired, and they can't fall back asleep; it’s a different use case. And then all of the combinations that occur in between. In other words, chronic pain or anxiety or whatever it may be. And would you use different types of products for a given condition or symptom?\n\n**CE:** Yes, for sure. So, you're talking about clinical manifestations of pain. There's inflammation in inflammatory and neuropathic conditions, and cannabis treats them slightly different. Maybe you have acute pain, so you want to vape. Either flower or concentrate because you need acute relief to help minimize the pain so you can fall asleep.\n\nOr maybe you can fall asleep, but you wake up in the middle of night. So, maybe you want an edible or some type of oral dosage as you fall asleep as their effects last longer than the effects from vaping.\n\n**Perry Solomon:** Yes, exactly. And it might be different for everybody. Maybe you vape to go to sleep; the vape helps you fall asleep. And then you also take a dosage of an edible, so it helps you maintain sleep throughout the night. And part of the journey that Chris was talking about is for people to keep track of their sleep cycles. Because what you eat has something to do with how cannabis, the medicine, reacts in your body. What time you took it before you went to sleep and how you're feeling the next day are also very impactful in terms of what you use.\n\nSomeone may wake up and say, I still feel a little bit high. I took an edible. Maybe I should have used that before—an hour or a half hour before I went to sleep. So, keeping a journal about what it is you took and how you slept that night kind of dials in for each person what works for them. Because everyone reacts differently to cannabis, and everyone goes a different way in their journey of going to sleep.\n\nOne of the biggest frustrations to be able to recommend something is that there's no one size fits all with cannabis. It's more about trying something, seeing what and how it works, and moving onto something else or going on to keep using the same product. Because everyone completely depends on their own personal physiology and what else they do during the day. Cannabis isn't _the_ answer. It's a part of a holistic approach that includes changing your lifestyle and eating habits as well as exercise.\n\n### Do physicians recognize cannabis as an effective treatment for insomnia?\n\n**PS:** No, doctors don't have any idea that cannabis works like that at all because no one taught us about cannabis in medical school. Physicians are very afraid of giving recommendations or talking about it mostly due to their ignorance. If you're going to ask your doctor about cannabis for insomnia, they'll look at you with a blank stare or say, “Well, let me give you some Ambien or some Xanax,” because they know how to prescribe that. They can tell you to take a half a pill every hour. Doctors know exactly how it's going to work.\n\nCannabis is a very individualistic thing, and it's frustrating for a physician not to know exactly how much to prescribe and write that out on their prescription pad. Doctors can write something for pain; they can write something for antibiotics, but they can't write a prescription specifically telling you exactly how much cannabis to take.\n\nThey're learning though. When you go in to talk to your physician about using cannabis, I highly recommend you show them what you’ve learned, because you will educate them about using cannabis—for insomnia specifically, and for other medical conditions. \n\n### So, how does someone know how much cannabis to take for their insomnia? \n\n **CE:** It depends on a person’s experience with cannabis—how much have you tried cannabis—and then understanding dosing and different routes of administration. Essentially, there are five routes of administration. You can have a topical route of administration. You can smoke or inhale. You can swallow things; that's an oral route of administration. You can let it dissolve in your mouth through sublingual or mucosal route of delivery; that's sublingual. There's also rectal and vaginal—or suppository.\n\nMost products on the market are topicals, inhalants, sublingual and oral. And each one has a different effect in your body, because your body processes cannabis differently depending on the route of administration or also because of the uptake for it. And so if people are unfamiliar with cannabis, it's important to understand the different routes of administration. Different products are formulated for different routes, and you're going to have different experiences depending on the route of administration.\n\nFor people who are new to cannabis, start low and go slow. Microdosing is very popular as a term; these are products that are 5 mg or less. If you're concerned about psychoactivity, start with something that’s high in CBD, or CBD only. There’s some other non-psychoactive cannabinoids out there, but CBD is usually a really good one to start with as the baseline. Then you can start to incorporate some delta-9 THC and find a balance between those two that may work for you. What would you suggest to someone like that? \n\n**CE:** For me, CBD works really well. There’s also a cannabinoid CBN. It's pretty rare. It's kind of hard to find because the plant actually doesn't produce it. It's a non-enzymatic degradation byproduct. So, it converts over time from delta-9 THC to CBN. Purportedly, it has very good sedating or sleep-enhancing properties. But we haven't really been able to look at it, and there's not a lot or very many products on the market that have it.\n\nSo, CBD is a great one to really look at. It helps people relax. A lot. It may calm your mind, reduce anxiety. There's also delta-8 THC that's starting to hit the market. This is a very relaxing cannabinoid, and we find that when combined with CBD, it has very relaxing qualities. I actually use a mixture of these two cannabinoids every night to sleep because it helps me calm, helps my mind stop racing.\n\nSo, you get into a pattern or ritual of going to sleep every night. You take some cannabis, whatever it is that works for you. You put your phone down since you’re trying to calm your body down and relax from the day, and then potentially use an oral dosage 30–60 minutes before you lay down.\n\nAnd when you’re lying down, maybe you use a tincture or some type of sublingual that's fast-acting, usually 5–15 minutes, that then helps this onset ramp before the oral dosage kicks in to help you maintain throughout the night. There's a ton of products out there that you can take that are between 5–20 mg, depending on what's going to work for the individual. \n\nAlso, can you identify parts of your day when you could incorporate breathing or meditation or supplement with some cannabis, too? During the day, we can get really stressed, whether we're dealing with some crisis or interacting with family or employees. We don't think about it. We just get through the day.\n\nSo, when we start shutting our bodies down, and we're in the middle of our sleep, we wake up in a panic, because now our bodies are trying to process all of these emotions from the day. So, can we identify throughout the day what's happening that leads to us having these persistent sleep issues, and then in that, can we use cannabis to help us address these issues?\n\nI think when we don't try to change patterns and adjust throughout the day, that's when we say, “Oh, I want a silver bullet.
