where is patient number

Hi there! On your digital or embossed copy of your recommendation, you can find you Patient Number listed just below your name, and above your date of isse. It will look something like this:

Patient Name: MaryJane Doe Patient #: ABC123 Date of Isse: 01/01/2017 Expires: 01/01/2018

You can also find your Patiente number listed on the back of your photo ID card. It is also listed abouve your Date of Birth on the front of your ID card.
